Are you an experienced and dedicated social worker passionate about improving the lives of adults in need of care and support? We have an exciting opportunity to join the Northeast Derbyshire Adult Social Work team, where you will work as part of a friendly, community-based, this team is focused on delivering high-quality care and support services to vulnerable adults and their careers.

In this pivotal role, you will carry out assessments and reviews in line with the Care Act (2014), ensuring that adults receive the support they need, to maximise their independence and well-being. The team operates with a flexible, agile working approach across Northeast Derbyshire, giving you the autonomy to manage your caseload in a way that suits your professional style and needs.

Key Responsibilities:

  • Conduct telephone and face-to-face Care Act assessments and reviews with a person-centred, strength-based approach.
    * Build positive, professional relationships with partner agencies such as Health, Police, Housing, and the PVi sector.
    * Demonstrate high levels of autonomy in case management, ensuring timely recording of interventions.
    * Provide support to carers and identify signposting opportunities to improve independence and well-being.
    * Manage a complex caseload while adhering to statutory duties and ensuring safe, cost-effective delivery of care.
    * Participate in Duty cover when required, triaging incoming work to meet urgent needs
    * Take the lead on Section 42 Safeguarding and chairing multi-agency meetings (MARM).
    * Complete Mental Capacity Act (2005) assessments, including Best Interest meetings, COP-DOLs, and Court work (with support and training available).

What do you need?

To be successful in the application process, candidates will need to have:

  • Qualified Social Worker experience, with experience in Adult Social Work and Care Act (2014) assessments
    * A strong understanding of relevant social work practice, ethics, and legislation
    * Proven ability to manage complex caseloads and prioritize work effectively
    * Experience building relationships with partner agencies and external stakeholders
    * Knowledge of Safeguarding Adults and the Mental Capacity Act (2005)
    * Strong communication skills and the ability to adapt in a fast-paced, dynamic environment
