We are seeking social care practitioners from either adult or children backgrounds to be part of Hertfordshire's 0-25 Together Service to help us deliver a practice model designed for the future.


0-25 Together is a new service launching in October 2016, to all disabled children, young people and young adults in Hertfordshire. We have a number of opportunities for adult and children practitioners to join this new service.

This is a specialist service for disabled children, young people, young adults and their families with a focus on supporting people to reach their full potential, enabling their independence and ensuring that they can take part in their local community. The service will support young people and young adults until they are settled in adult life.

As a Family Practitioner you will support families with children from birth to 13 years. You will provide appropriate responses to children, young people, young adults and their families in need of support and taking appropriate safeguarding action as and when required.

We are looking for experienced practitioners who understand the needs of disabled children and young people and their families to be part of building this exciting new team.

We are looking for you if:

* You are prepared to work in new ways and adapt your approach to practice in order to deliver our new service model
* You are aware of recent legislative reforms and will be ready to work with partners in health and education to deliver the support that our customers need

Although there are no specific qualifications required for these roles previous experience in working with children and families would be beneficial.
