The role

Action Breaks service provides a holiday opportunity for children and young adults between the ages of 5 - 18 years old with various needs including learning disability and complex health needs who live in Bristol and currently do not receive the overnight short break service. Action Breaks is an OFSTED regulated service who are proud of their 'Good' rating.

Action Breaks holds five short breaks a year- Each break consists of a two-night stay for Children and Young people only. Children and Young people will be assessed before the breaks and will be matched to a specific holiday to meet their individual needs.

As a Short Breaks Holiday Leader you'll lead the team to provide a safe, stimulating, fun and caring environment, supporting the children with their social and emotional needs, social skills, independence and promoting life skills, as well as all aspects of their care. You'll also lead team meetings and supervision during the day.

You will make a difference by:

- Taking a lead with case/care planning management for individual children or young people, undertaking assessment of need, developing a care plan and identifying agreed outcomes and carrying out regular review in conjunction with children, young people and their families and other professionals.

- Leading short break holidays with accountability for service delivery, assessing and managing risks, deploying staff and resources accordingly while on the shift and to maintain personalised high quality care that is outcome focused for children and young people and apply agreed organisational procedures to ensure the safe care and management of children and young people in our care.

- Supervising, guiding or directing staff and volunteers where required and provide mentoring to newly appointed staff.

- Undertaking assessment of children and young people coordinating multi agency and family information, analysing and producing reports; attendance at professional meetings including reviews, court work as well as promoting the Service in visiting Schools and Community meetings to share news of upcoming events and referral process, as well as meeting with Commissioners to review the service delivery.

- Strong organisational skills with the ability to multi task and prioritise workload.

- Natural ability to build rapport with others, Developing effective working relationships with team members, partner agencies, other external agencies and Action for Children colleagues.



You will need:

- Essential to have NVQ Level 3 in Health and Social Care or QCF Level 3, or the willingness to complete this within 18 months of starting the role.

- Experience of working in a residential setting for children and young people.

- Experience of working with Young people and Children living with a disability,

- Motivation and energy to support young people to take part in activities and attend Education.

- Excellent communication skills.

- Good knowledge of IT and ability to work confidently using it.

- Experience of being a key/link worker.

- Good Knowledge of Safeguarding and keeping young people safe.

- Access to a car with business insurance. The job requires the ability to travel according to the needs of the job, with reasonable adjustments, if required, according to the Equality Act.
