Brent Council is seeking a skilled Adult Social Worker to join its Hospital Discharge Team, supporting adults to move safely and confidently from hospital back into the community.
This role is ideal for a practitioner who thrives in a fast-paced environment and is passionate about person-centred, outcome-focused practice.
About the Role
Facilitating safe and timely hospital discharges for adults
Carrying out Care Act 2014 assessments and reviews
Coordinating care and support packages tailored to individual needs
Working collaboratively with NHS colleagues, families and partner agencies
Promoting independence, wellbeing and choice for service users
Managing cases effectively within a hospital setting
