Community LD nurse for 0-25 working within the integrated team. Working with young adults age 18+


It is a community role working Monday to Friday 9am to 5.30 so yes will need to be a driver. Some capacity for WFH admin but will need to be available to be out on visits

Purpose of the Job

To support the development of partnerships with key health providers in Hertfordshire to support a smooth transition of healthcare provision from children's health to adults health services.

To promote equitable health outcomes for young people aged 18-25 years with learning disabilities and highlight areas of inequality to commissioners of services.

To assist Hertfordshire Health Care Providers to identify training needs and to deliver training programmes designed to meet identified needs regarding facilitating health for adults with learning disability.

Main Areas of Responsibility

Clinical

To undertake comprehensive assessments of for referrals made to the 0-25 Together Learning Disability Nursing Service.

To carry a complex caseload as an allocated nurse to service users referred to the0-25 Together Learning Disability Nursing Service.

To provide an individual consultative and/or advisory response to complex service user issues within primary, secondary and tertiary health care settings.

To support the utilization of individual Health Action Plans within health and social care settings across Hertfordshire.

To support the development of robust communication systems that promotes effective and efficient transfer of care between children's to adult health services.

To signpost service users, carers, professionals and others to appropriate health services/resources.

To identify need for Joint assessments with other agents and support these as appropriate.

To participate in special interest groups, seminars, topics to improve practice

.

To promote the use and development of 'My Purple Folder', identifying health needs and promoting access to Primary, Secondary and Tertiary care settings.

To plan, develop and implement a programme of education in collaboration with HCC Learning and Development Team and support the delivery of training to partner agencies in health and education settings.

To promote the health, wellbeing and safety of vulnerable service users whilst in

Primary, Secondary and Tertiary health care settings.
