The role
Our Fostering South West Peninsula service provides stable homes for children and young people who cannot live with their families. Our Family Breaks Devon service provides short breaks for children and young people with additional needs, most of whom live with their parents. Within these services, we have specific support from a corporate partner, CBRE, to hire a practitioner who will work directly with children aged 15-25 years as they move from childhood to independent living. We are seeking a Transitions Worker to join our team on a Fixed-Term contract working 22.2 hours week.
As the Transitions Worker your role will be to provide 1:1 practical and emotional support to young people within the programme. This will include assisting young people to secure suitable accommodation where needed, build aspirations and find suitable employment and training, education opportunities.
The service locally needs you to provide good collaborative work with young people, and with our local authority partners. The local authorities develop and review plans for each child and young person with the individual and with other professionals. We cover a large region, to Cornwall and works with all the local authorities in this area. We have a number of young adults who are leaving the care system but who have chosen to Stay Put from 18-21 years with their former foster carers. Our young people with additional needs, who receive short breaks, have a right to support up to age 25 and we will particularly welcome applicants who have an interest or willingness to learn about disability and ensure that the young adults' voices are heard in planning their future living.
You will make a difference by:
- Delivering complex and specialist interventions with children, young people or families on a 1:1 basis, group or community setting.
- Attending and participating at relevant multi agency meetings, providing a lead role ensuring reflective and analytical practice review.
- Providing a high-quality service to children, young people and their families, which is based on an assessment of need and is outcomes driven.
- Helping change the lives of the young people and their foster carers.
You will need:
- Level 3 NVQ in Childcare of Youth work or equivalent.
- Previous experience having worked with children in care or leaving care.
- Working knowledge of how the care system works is desirable.
- Previous experience having worked with the age range of 15-25 years old.
- Good IT skills is desirable due to the nature of the role and data input required from reports.
