The role
Action for Children Clackmannanshire Families Support Service provides early intervention and support to children and their families with the key aim of improving outcomes for children who would otherwise not reach their full potential and who may then require more complex interventions.
Clackmannanshire Family Support Service are also proud to be working in partnership with NHS Forth Valley to deliver a Child Healthy Weight Early Intervention and Prevention Project.
We are looking for a Family Support Practitioner to join our team. This service is specifically intended for children from prebirth to two years of age and their families. The Aims of the service include ensuring where possible Children at age two are within the perimeters of growth charts, Parents are empowered with the skills and knowledge to prevent childhood obesity, Parents have a positive and supportive experience, Positive behaviour change concepts (BCC) are triggered to enable sustained changes and Parents have increased /maintained activity levels (i.e. continue walking with children).
You will make a difference by:
- Having the skills, confidence and knowledge to support children and families to eat well and have a healthy weight.
- Educating parents to feel better equipped and confident to provide their children with a healthy start, including good nutrition and healthy eating habits.
- Encouraging more families at risk of diet-related health inequalities to engaging with services and accessing support.
- Providing a range of outcomes focussed group and individual support packages to vulnerable families.
- Working in partnership in a person centred way with children and families in order to improve outcomes regarding their wellbeing, confidence, ability and awareness.
You will need:
- Experience of Working within a multi-agency social care, early years or health environment and applying safeguarding procedures.
- Experience of managing cases, in accordance with agreed procedures and completing assessments, planning and reviews to ensure positive outcomes.
- Working knowledge of an outcome-based approach to planning, delivering and reviewing services.
- Assessment, Service Planning and Delivery experience.
- A driving license and access to a vehicle is desirable.
