๐ Hours: Mon-Thurs 9:00-17:30 | Fri 9:00-16:30

๐ About the Role:
Join our dedicated Older People's Social Work Team as a Social Worker, supporting adults across East Herts, Broxbourne, and Welwyn Hatfield. This 6-month contract is funded through winter pressures, offering a valuable opportunity to make a meaningful impact during a critical time.
๐ง Key Responsibilities:
๐ Based in the locality, supporting people in their own communities
๐ฅ Help reduce hospital admissions through timely intervention
๐ Complete Care Act assessments and reviews
๐งฉ Arrange and coordinate appropriate care and support packages
๐ก๏ธ Act as Safeguarding Enquiry Officer
๐ง Undertake Mental Capacity Assessments and Best Interest decisions
๐ค Work collaboratively with professionals across health and social care
โ What We're Looking For:
๐ Qualified Social Worker
๐งพ Registered with Social Work England
๐ Full UK driving licence
๐ Strong organisational, communication, and IT skills
๐ต Experience working with older adults, including those with cognitive impairments
