๐งฉ About the Role:
We are looking for a dedicated and experienced Adult Social Worker to join our team in London. You'll be supporting adults in need, promoting independence, and ensuring safeguarding principles are met while working collaboratively with multidisciplinary teams.
๐ง Key Responsibilities:
๐ฅ Conduct assessments of adults and older people in line with statutory duties.
๐ก Develop and implement care and support plans to promote independence and well-being.
โ๏ธ Safeguard vulnerable adults and ensure compliance with the Care Act 2014.
๐ค Work collaboratively with health professionals, service providers, and families.
๐๏ธ Maintain accurate case records and documentation.
๐ฌ Provide advice, advocacy, and emotional support to service users and carers.
โ Requirements:
๐ Qualified Social Worker (Degree/DipSW/CQSW).
๐ชช Current Social Work England registration.
๐งพ Experience working within Adult Services (e.g., safeguarding, hospital discharge, community teams).
๐ Ability to travel within the borough when required.
